Description • Receive First Link® referrals and perform intake functions including establishing client contact, evaluating services and making referrals to internal and external programs.
• Provide supportive counseling through individualized and/or group support sessions.
• Develop and coordinate specialized education and training programs for people living with ADRD and their families using standardized materials from the First Link Learning Series.
• Advocate for improved services for people with dementia, where appropriate.
• Occasional travel to Algoma District (Batchewana Bay to Thessalon).
• Administer cognitive assessments using standardized tools.

Qualifications:

1. Post-secondary education in a health and/or social service field and current membership in a regulated health profession (eg nursing, social work).
2. Minimum 3 years of experience working with people with dementia and their families.
3. Previous experience or knowledge of dementia and its impact on the family unit.
4. Knowledge of community health care organizations.
5. Current standard first aid/CPR certification.
6. Excellent written and verbal communication skills.
7. Demonstrated organizational skills.
8. High level of proficiency with Microsoft Office applications.
9. Valid driver’s license and regular access to a vehicle.

Responsibilities:

1. Provide education and support for people with dementia through home or office visits and by telephone.
2. Arrange and/or facilitate support group meetings for family members (including children) and caregivers as well as people with dementia. Examples include: First Link Learning Series, Caregiver Information and Discussion Group, Living Well with Memory Loss and other support groups as needs are identified.
3. Receive and refer referrals from primary care practitioners and other community support services; for example, Ontario Health home and community care services, adult day programs, mental health services for seniors, group health center programs, etc.
4. Advocate on behalf of people with dementia for improved services, when needed and/or requested.
5. Maintain up-to-date confidential client records, including a written record of all services provided to clients and their caregivers.
6. Implement and monitor program standards and appropriate Alzheimer Society policies and procedures.
7. Refer to volunteer visiting program, including volunteer orientation assistance, volunteer matching support, service support.
8. Promote Medic Alert Safely Home™ and participate in the registration process as needed.
9. Recommend additional resources for the Katherine L. Punch Resource Center.
10. Represent the Company on community and/or internal committees, when requested.
11. Perform public speaking duties when requested, particularly in relation to direct customer service.
12. Provide family support services to communities surrounding Sault Ste. Marie, from Thessalon to Batchewana Bay.
13. Participate in Company-sponsored professional development opportunities as identified (may require travel to Southern Ontario).
14. Monitor program expenditures related to travel to ensure they remain within the approved budget.
15. Record monthly program statistics and submit monthly to the General Manager.
16. Provide backup in the absence of the Education Coordinator.
17. Take responsibility for personal improvement in knowledge of Alzheimer’s disease and related dementias, as well as ongoing professional development.
18. Attend the Annual General Meeting.
19. Participate in fundraising activities as directed by the Executive Director.
20. Perform other duties as may be assigned by the General Manager.
